Responsibilities

• Assesses and treats patients; designs a plan of care with goals, treatment, outcomes, and discharge plan
• Performs patient examinations and uses reliable outcomes measures to reflect progress
• Educates patients, families, and other health professionals using effective teaching methods
• Monitors and documents patient progress and modifications to treatment plans
• Supervises COTA and rehabilitation technicians and provides feedback to the care team
• Communicates discharge disposition and equipment needs to the medical team and family
