Qualifications

• BLS certification within 30 days (AHA).
• Licensed Occupational Therapist (OT) – IL.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Uses safe body mechanics and develops an appropriate plan of care including goals, treatment, outcomes and discharge plan.
• Educates patients, family, caregivers, students and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Documents evaluations and plans of care in accordance with department policy.
