Job summary

Evergreen Retirement Community in Oshkosh, WI is seeking a skilled Occupational Therapist (OTR) to join our dynamic rehab team on a PRN basis. You will assess, plan, and implement individualized therapy programs for short-term rehab patients and long-term residents, helping them regain or maintain independence in daily living.

The ideal candidate will hold a Bachelor’s or Master’s degree in Occupational Therapy, current state licensure (OTR/L), and demonstrate excellent communication,

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Current state licensure as an Occupational Therapist (OTR/L).
• Excellent communication, organization, and time management skills.
• Compassionate, patient-centered mindset with a passion for helping others.

Responsibilities

• Evaluate patients’ functional abilities and develop individualized treatment plans.
• Provide hands-on therapy to improve residents’ and patients’ ability to perform daily activities (ADLs).
• Document progress and adjust treatment plans as needed.
• Educate patients, residents, and caregivers on exercises and strategies to improve function.
• Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers.
• Ensure compliance with regulatory requirements and documentation standards.
• Maintain accurate records using electronic medical record (EMR) systems.

In this role, you will assess, plan, and implement individualized therapy programs for both short-term rehab patients and long-term care residents.
