Job summary

Greenfield Rehabilitation Agency is seeking a Part-Time or PRN Occupational Therapist Registered (OTR) in Port Edwards, WI. Join a passionate therapy team dedicated to improving the lives of seniors. Qualifications

• Graduate of an accredited school with a bachelor's, master's, or doctoral degree in Occupational Therapy.
• License as required by the State of Wisconsin to practice as an Occupational Therapist.

Responsibilities

• Complete evaluations assessing residents’ functional, cognitive, and physical abilities.
• Develop and implement individualized, goal-driven treatment plans.
• Provide OT interventions focused on ADLs, self-care, upper extremity strength, and functional mobility.
• Educate residents, families, and staff on therapy goals and strategies.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Maintain timely and accurate documentation per regulatory standards.
• Promote a safe, encouraging therapeutic environment that supports resident independence.
