Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team. The clinician will assess patients requiring rehabilitation, develop and administer treatment programs, and document progress while educating patients and families.

The OT will supervise Occupational Therapy Assistants and collaborate with all disciplines to ensure the highest quality patient care. Responsibilities include goal-driven therapy plans, adherence to HIPAA, and timely

Qualifications

• Graduated from accredited OT program.
• Must hold valid OT license and registration.
• Experience with long-term/geriatric care preferred.
• Supervisory responsibilities of OTAs.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment plans with goals based on patient capacity and tolerance.
• Document patient progress and educate patients and families in therapy methods.
• Collaborate with all disciplines to provide best patient-centered care.
• Report to Director of Rehabilitation and adhere to established schedules and policies.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Compensation: $43-47/hour
