Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team in a subacute care setting. You will assess referred patients, develop individualized treatment plans, and supervise Occupational Therapy Assistants.

The role emphasizes documentation, collaboration with a multidisciplinary team, and commitment to patient-centered care. A $5,000 sign-on bonus is offered for qualified candidates.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid state license/registration for practice.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.
• Oversee and supervise OTAs.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document and record patient condition; educate patients and families.
• Collaborate with other disciplines to deliver patient-centered care.
• Oversee OTAs and supervise support staff; ensure compliance with HIPAA and confidentiality.
• Prepare evaluations, progress notes, re-certification and discharge documentation.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Compensation: $43-47/hour
