Job summary

The Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join a dedicated rehabilitation team in New Jersey. The OT will assess referred patients, develop and administer individualized treatment plans, and educate patients and families on therapy methods while supervising assistants.

A $5,000 sign-on bonus is offered. Qualifications include an accredited OT degree, current state license, and experience in long-term or geriatric care preferred.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Must hold a valid state license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation skills including weekly notes and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Maintains confidentiality and HIPAA compliance.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and assess medical condition, functional abilities, and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Document patient condition and educate patients and families on therapy methods.
• Oversee and supervise Occupational Therapy Assistants.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Compensation: $43-47/hour
