Zusammenfassung

Homeside Rehab is seeking compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to join our growing home health team across Northern New Jersey. Deliver patient-centered OT services in patients' homes, focusing on ADLs, upper extremity function, and functional independence.

You'll evaluate, plan, and implement evidence-based interventions, document accurately in electronic systems, and collaborate with physicians and caregivers.

Qualifikationen

• Active New Jersey OT or COTA license required.
• Minimum 1 year of experience in Home Health, Outpatient, EI, or SNF preferred.
• Excellent clinical assessment and communication skills.
• Ability to work independently in home health setting.
• Valid New Jersey driver’s license and reliable transportation.

• Perform comprehensive occupational therapy evaluations and develop individualized treatment plans based on each patient's needs and goals.
• Provide evidence-based occupational therapy interventions to improve ADLs, fine motor skills, upper extremity function, coordination, sensory processing, and overall functional independence.
• Deliver skilled therapy services in patients' homes while promoting improved function and quality of life.
• Complete accurate, timely, and compliant electronic documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.
• Educate patients and caregivers on home exercise programs, adaptive equipment, and therapeutic techniques.
• Communicate effectively with office staff regarding scheduling, patient progress, and clinical needs.

• Excellent clinical assessment and communication skills

• Ability to work independently in a home health setting

• Basic computer proficiency for electronic documentation

• Valid New Jersey driver's license and reliable transportation

Position Details

• Job Types: Full-time, Part-time, Per Diem

• Compensation: $65.00–$80.00 per hour (based on experience)

• Schedule:

  • Choose your own hours

  • Monday through Friday

  • Day and evening shifts available

  • Weekend availability optional
