Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist to join an in-house Skilled Nursing Facility partner in upstate New York. You will design, implement, and monitor therapy plans to promote resident independence and quality of life.

The role supports full-time, part-time, or PRN scheduling with a relocation bonus offered for eligible candidates; must hold an active OT license in the practicing state and commit to ongoing professional development and compliant documentation.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Commitment to continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screenings of residents to determine best interventions.
• Evaluate residents to inform treatment plans and set goals.
• Interpret and communicate evaluation findings and treatment plans to residents and families.
• Establish individualized therapy schedules with the team.
• Conduct therapy according to treatment plans and document/bill appropriately.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time (Relocation Bonus!), Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
