Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Tupper Lake, NY. Full Time, Part Time, or PRN options with a sign-on bonus may be available.

The role involves planning, organizing, and directing OT services to ensure high-quality resident care, compliance with standards, and collaboration with the interdisciplinary team. Active licensure and ongoing education are required.

Qualifikationen

• Ongoing continuing education to maintain licensure and certification.

Aufgaben

• Conduct regular screenings to identify best interventions for each patient.
• Develop and adjust treatment plans with short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and team.
• Schedule resident therapy in coordination with the team.
• Provide therapy per plan and ensure proper documentation/billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet standards.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time (Sign On Bonus!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient




• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals




• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members




• Establish individual resident therapy schedules in cooperation with other team members




• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services




• Perform all other duties as assigned and as required to effectively treat patients




• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy




• MUST have an Active Occupational Therapy license in practicing state(s)




• Ongoing continuing education and professional development to maintain current licensure and certification
