The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking an Occupational Therapist to join our team in Brooklyn, NY. The OT will assess patients, develop treatment plans, and work within a multidisciplinary team to optimize functional outcomes.

Applicants should be licensed in the state, experienced in geriatric care, and proficient in clinical documentation and patient education. The role requires professionalism and timely communication with the rehabilitation team.

Formación

• Must have graduated from an accredited OT program.
• Must hold a valid state license to practice.
• Experience supervising Occupational Therapy Assistants and coordinating care with other disciplines.

Responsabilidades

• Assess referred patients for rehabilitation needs and determine eligibility.
• Develop and administer treatment plans with goals based on patient capacity.
• Document patient condition, progress and discharge planning.
• Educate patients and families on therapy methods and expectations.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
• Report to the Director of Rehabilitation and adhere to schedules.
