Job summary

Trinity Health At Home is seeking a Home Care Occupational Therapist to deliver compassionate, in-home therapy across communities in Michigan. You will assess function, craft individualized plans, and guide clients toward greater independence while collaborating with physicians and care teams.

You must be a graduate of an accredited OT program with NBCOT certification and state licensure, plus at least 1 year of experience and a valid driver’s license; home health experience is preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state licensure/registration.
• Minimum one year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.

Responsibilities

• Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
• Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
• Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
• Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
• Coordinate care with interdisciplinary teams and participate in case conferences.
• Document all care activities accurately using point-of-care systems.

Position Purpose

What You Will Do
