Job summary

Trinity Health At Home in Michigan is seeking a dedicated Home Care Occupational Therapist to deliver client-centered therapy in patients' homes. You will assess function, design personalized plans, and promote independence through therapeutic interventions.

Join a multidisciplinary team, supervise COTAs and HHAs, document care activities, and contribute to care coordination across the region. A valid OT license and NBCOT certification are required; home health experience preferred.

Qualifications

• Graduate of an accredited OT program.
• NBCOT certification and state licensure required.
• Minimum one year of experience or 6–8 week preceptorship.
• Valid driver’s license and reliable transportation; home health experience preferred.

Responsibilities

• Assess clients’ functional status and develop individualized treatment plans with physicians.
• Deliver preventive, therapeutic, and rehabilitative services per the plan of care.
• Educate clients and families on safety, health promotion, and prosthetic/orthotic device use.
• Supervise COTAs and HHAs.
• Coordinate care with interdisciplinary teams and participate in case conferences.
• Document all care activities accurately using point-of-care systems.

Position Purpose:

What You Will Do:

• Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.

• Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.

• Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.

• Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).

• Coordinate care with interdisciplinary teams and participate in case conferences.

• Document all care activities accurately using point-of-care systems.

Minimum Qualifications:

• Graduate of an accredited Occupational Therapy program.

• NBCOT certification and state licensure/registration.

• Minimum one year of experience or completion of a 6–8 week preceptorship.

• Valid driver’s license and reliable transportation.

• Home health experience preferred.
