Job summary

Trinity Health At Home is seeking a Home Care Occupational Therapist to deliver in-home therapy services, helping clients regain independence through individualized plans and interventions.

You will assess, treat, educate, and coordinate with interdisciplinary teams, supervising COTAs and HHAs. A NBCOT and state licensure are required; home health experience preferred. Driving required; day shift. Sign-on bonus up to $10,000.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state licensure/registration.
• Minimum one year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.
• Commitment to ethical standards and continuous learning.

Responsibilities

• Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
• Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
• Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
• Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
• Coordinate care with interdisciplinary teams and participate in case conferences.
• Document all care activities accurately using point-of-care systems.

What You Will Do
