Job summary

Susquehanna Health and wellness center is seeking a highly skilled Occupational Therapist (OT) or Occupational Therapist Assistant (COTA) for a Per Diem, flexible schedule. Join our care team to help improve the quality of life for residents at our large rehab and nursing facility in New York.

Your role focuses on planning, organizing and delivering OT services in line with federal, state and local standards to maintain high resident care quality. Weekday and weekend availability is requested.

Qualifications

• Master's degree in OT required.
• Associate's degree in COTA required.
• Minimum of one year of related experience in a hospital, skilled nursing facility, or similar setting.
• Ability to read, write, speak and understand English.

Responsibilities

• Provide individual therapy services to residents according to care plans.
• Assist in resident assessment activities as needed.
• Assist in developing therapy plans for residents with MDS.
• Adjust therapy plans to support resident care goals.
• Brief residents on procedures involved in occupational therapy.

As our PRN OT/COTA, your primary focus will be to assist in the planning, organizing, developing and delivering of Occupational Therapy Services in accordance with current applicable federal, state and local standards, guidelines and regulations, to ensure the highest degree of quality resident is continuously maintained.

Duties and Responsibilities Include:

Providing individual therapy services to residents according the care plans.

Assisting in resident assessment activities as needed

Assisting as needed in developing therapy plans for individual residents in conjunction with MDS.

Keeping the Therapist informed of any need to adjust therapy plans to match or support resident care plan goals.

Briefing residents in procedures involved in occupational therapy and preparing necessary equipment.

Participating in discharge planning, development and implementation of resident care plans, resident assessments, etc.

Interviewing residents and family members as necessary.

Coordinating therapy appointments with other departments.

Following therapy department safety standards.

Maintaining the care and use of supplies, equipment and appearance of the therapy area.

Therapy requirements include the ability to lift 5 LBS - 45 LBS unassisted and intermittent movement throughout the work day.

Weekday and weekend help needed

Qualifications

A minimum of a Master's degree from an accredited college or university for OT is required.

An Associate's Degree from an accredited college or university for COTA is required

A minimum of one year of related experience in a hospital, skilled nursing facility or other related medical facility is required, as is the ability to read, write, speak and understand the English language.

The successful candidate will also demonstrate patience, courtesy, excellent customer service skills, dependability, attention to detail and flexibility.
