Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a compassionate PRN Occupational Therapist to join our rehabilitation team in a SNF setting. The OT will provide skilled therapy services to promote independence, mobility, safety, and quality of life.

Responsibilities include evaluations, treatment plans, discharge planning, documentation, and collaboration with the interdisciplinary team to ensure resident-centered care.

Qualifications

• Active Florida OT license.
• Minimum 3 years of OT experience in a Skilled Nursing Facility (SNF).
• Strong knowledge of long-term care, rehabilitation, and skilled nursing practices.
• Excellent communication, documentation, and clinical skills.
