Job summary

Chatham Hills Subacute Care Center in Whippany, NJ, seeks a licensed Occupational Therapist to assess patients, design therapy plans, and contribute to a multidisciplinary rehab team in a subacute setting.

Responsibilities include documenting progress, educating patients and families, and supervising OT Assistants to ensure high-quality, individualized care for residents requiring rehabilitation services.

Qualifications

• Must have graduated from an accredited OT program.
• Must hold a valid professional license for the state of practice.
• Experience with long-term care is preferred but not required.
• Strong documentation and record-keeping skills.
• Ability to supervise OT Assistants and collaborate with a rehab team.
• Commitment to patient-centered care and service excellence.

Responsibilities

• Assess referred patients to determine rehabilitation needs and functional status.
• Establish and administer treatment plans with goals aligned to patient capacity.
• Document patient condition, progress, and discharge plans.
• Educate patients and families on therapy methods and home programs.
• Collaborate with interdisciplinary teams to ensure best patient care.
• Supervise Occupational Therapy Assistants and ensure quality care.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
