Job summary

Chatham Hills Subacute Care Center is seeking a skilled Occupational Therapist to join a growing rehabilitation team in New Jersey. You will assess referred patients, develop and implement personalized treatment plans, and document progress under the direction of the Physician and Director of Rehabilitation.

The role includes supervising OT Assistants, coordinating with other disciplines to deliver patient-centered care, and ensuring accurate discharge documentation while maintaining HIPAA

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Valid OT license and state registration.
• Experience supervising OT Assistants preferred.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills.
• Ability to write evaluations, progress notes, re-certifications, and discharge documents.
• Collaborates with disciplines to provide patient-centered care.
• Exhibits professional conduct and exceptional patient service.

Responsibilities

• Assess referred patients for rehabilitation needs and determine functional capabilities.
• Develop and administer individualized treatment programs with clear goals.
• Document patient condition and progress; educate patients and families.
• Supervise Occupational Therapy Assistants.
• Collaborate with other disciplines for integrated care.
• Prepare discharge documentation and weekly progress notes.
• Maintain confidentiality and HIPAA compliance.
• Report to Director of Rehabilitation and adhere to schedules.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
