Job summary

Chatham Hills Subacute Care Center is seeking a licensed Occupational Therapist to join our rehabilitation team in Summit, NJ. The OT will assess patients, develop and manage treatment programs, and document patient progress while educating families.

The role includes supervision of OT Assistants and collaboration with other disciplines to provide comprehensive, patient-centered care. Candidates should have graduated from an accredited OT program, hold state licensure, and demonstrate strong

Qualifications

• Must have graduated from an accredited OT college or university.
• Must hold a valid OT license/registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals tailored to patient capacity.
• Document and record patient condition and progress; educate patients and families.
• Collaborate with all disciplines to deliver patient-centered care.
• Ensure compliance with confidentiality, HIPAA, and residents' rights.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
