The Chatham Hills Subacute Care Center seeks an experienced Occupational Therapist to join its rehabilitation team in New Jersey. The OT will assess patients requiring rehabilitation, develop and supervise treatment plans, and document outcomes under physician direction.

Responsibilities include setting realistic goals, educating patients and families, and collaborating with a multidisciplinary team. Licensure in the state and accreditation are required, with experience in long-term or geriatric

Competenze

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid license/registration for the state.
• Oversee and supervise Occupational Therapy Assistants.
• Experience in long-term/geriatric care is preferred.
• Strong documentation writing skills.
• Experience with evaluations, weekly progress notes, re-certification, and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service skills and clinical excellence.
• Punctual, professional, and adherent to schedules.
• Perform duties assigned by the Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
