Job summary

The Chatham Hills Subacute Care Center is seeking an experienced Occupational Therapist to join our rehabilitation team. The OT will assess referred patients, develop treatment programs, and set goals aligned with physician guidance.

Responsibilities include documentation, weekly progress notes, discharge planning, educating patients and families, supervising OT Assistants, and collaborating with disciplines to provide patient-centered care in a long-term/geriatric setting.

Qualifications

• Must have graduated from an accredited OT college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants is required.
• Experience in long-term or geriatric care preferred.
• Strong documentation and record-keeping skills.

Responsibilities

• Assess referred patients requiring rehabilitation and determine functional status.
• Establish and administer treatment programs with goals per physician guidance.
• Set realistic goals and document patient progress and conditions.
• Educate patients and families in therapy methods.
• Supervise OT Assistants and collaborate with other disciplines to deliver patient-centered care.
• Complete evaluations, weekly progress notes, re-certification, and discharge documentation.
• Adhere to confidentiality, HIPAA, and residents' rights.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
