Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Minnesota. You will plan, organize, develop, and direct OT services to ensure high-quality resident care and compliance with applicable standards.

You will screen and evaluate residents, develop and adjust treatment plans, and coordinate with families and an interdisciplinary team. Competitive pay and growth opportunities are offered with full-timeHours.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to identify best interventions and treatment for each patient.
• Evaluate residents and develop treatment plans with goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Provide therapy per established plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments are carried out with proper reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
