Job summary

Evolve Therapy Services is seeking an Occupational Therapist to lead therapy services in an in-house skilled nursing facility. The role includes planning, directing, and evaluating occupational therapy for residents, with collaboration across the care team.

Active OT licensure in the practicing state is required, along with ongoing professional development to maintain credentials. Full-time, part-time, or PRN positions are available to fit staffing needs and resident care goals.

Qualifications

• Must hold a Degree in Occupational Therapy and an active OT license in the state of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to identify best intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret evaluation findings and communicate with residents, families/caregivers, and the interdisciplinary team.
• Establish individual therapy schedules with other team members.
• Conduct therapy per established plan and document/bill OT services.
• Oversee COTA and ensure documentation and treatments meet standards.
• Perform other duties as assigned to effectively treat patients.

Job description

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
