Job summary

Evolve Therapy Services seeks a licensed Occupational Therapist for an in-house Skilled Nursing Facility in Durham, NC. The role supports patient-centered care, plans and organizes OT services, and collaborates with an interdisciplinary team to maintain high-quality resident outcomes.

Positions available include Full Time, Part Time, or PRN. Ongoing professional development is encouraged, with emphasis on compliant documentation, reimbursement practices, and patient advocacy within the facility

Qualifications

• OT degree required and current licensure in NC.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Evaluate residents to inform treatment plans and set goals.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
