Job summary

Evolve Therapy Services in Ahoskie, NC is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility. The practice offers full-time, part-time, or PRN positions with responsibilities to plan, organize, and direct OT services in the facility.

You will screen residents, develop treatment plans, coordinate with the interdisciplinary team, supervise COTA, document care, and ensure compliant, high-quality resident care.

Qualifications

• Must hold an Occupational Therapy degree and be licensed to practice in NC.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to develop treatment plans and goals.
• Communicate evaluation findings and plans to residents, families, and the team.
• Establish resident OT schedules with the team.
• Document and bill OT services per plan.
• Oversee COTA and ensure compliant documentation and reimbursement.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
