Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Oklahoma City, OK. This full-time position focuses on planning, directing OT services and collaborating with an interdisciplinary team to deliver high-quality resident care.

Responsibilities include screenings, evaluation-driven treatment plans, goal development, and compliant documentation and billing. Candidates must hold an OT degree and an active license.

• Active OT license in practicing state(s)
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to determine best intervention
• Evaluate residents to inform and develop treatment plans with goals
• Interpret and communicate evaluation findings to residents, families, and team
• Establish individual therapy schedules with team
• Conduct therapy per treatment plan and ensure proper documentation and billing
• Perform all duties to effectively treat patients
• Oversee COTA and ensure documentation and reimbursement practices are followed

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
