Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Temple, OK. This PRN role focuses on planning, organizing, developing, and directing occupational therapy services in the facility to ensure high-quality resident care.

The ideal candidate holds an OT degree and an active license, with ongoing CE to maintain licensure. Responsibilities include evaluating residents, creating treatment plans, coordinating with the interdisciplinary team,

Qualifications

• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure.
• Degree in Occupational Therapy from an accredited program.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents to inform and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Schedule resident therapy in coordination with team members.
• Document and bill OT services per plan and progress.
• Oversee COTA and ensure documentation and treatments meet standards.
• Ensure clinical excellence and proper reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
