Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Lawton, OK. The role is PRN and focuses on delivering high-quality occupational therapy services to residents in a facility setting.

Responsibilities include screening, evaluation, treatment planning, scheduling, documentation, and collaboration with the care team. Must hold an active OT license in the practicing state and engage in ongoing professional development.

Qualifications

• Degree in Occupational Therapy (OT).
• Active Occupational Therapy license in the practicing state(s).
• Commitment to ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine optimal interventions for each patient.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual therapy schedules in cooperation with other team members.
• Deliver therapy per the treatment plan and ensure proper documentation and billing.
• Perform all duties to effectively treat patients as assigned.
• Oversee COTA as needed and ensure documentation and treatments meet clinical and reimbursement standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
