Evolve Therapy Services seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Temple, OK. This PRN role focuses on planning, directing, and delivering occupational therapy services to ensure high-quality resident care.

The candidate will screen residents, develop treatment plans, communicate with families and the interdisciplinary team, and coordinate therapy schedules with team members. Licensure and ongoing CE are required.

Formación

• Active OT license in practicing state(s) required.
• Ongoing continuing education and professional development to maintain licensure.

Responsabilidades

• Conduct regular screenings to determine best interventions.
• Evaluate and develop treatment plans with short- and long-term goals.
• Interpret evaluation findings and communicate plans to residents and families.
• Establish resident therapy schedules with the team.
• Perform therapy according to plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure clinical excellence and proper reimbursement.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
