Job summary

Evolve Therapy Services in Rice Lake, WI is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility on a PRN basis. You will plan, develop, and direct OT services, collaborating with an interdisciplinary team to ensure high-quality resident care and accurate documentation.

The role requires an OT degree, active licensure in the practicing state, and ongoing continuing education.

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents, families, and team members.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to plan and ensure documentation and billing.
• Oversee COTA and ensure documentation and treatments meet clinical standards and reimbursement practices.
• Perform other duties as assigned to treat patients.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
