Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Sutherland, NE. This PRN role focuses on planning, organizing, developing, and directing OT services to ensure high-quality resident care in compliance with relevant standards.

Responsibilities include screening residents, evaluating and developing treatment plans, communicating findings to the care team and families, scheduling therapy, documenting and billing, supervising COTA as needed,

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s) is mandatory.
• Ongoing continuing education to maintain licensure and certification is expected.

• Evaluate residents and develop short- and long-term goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and care team members.
• Establish individual resident therapy schedules in coordination with team.
• Conduct therapy according to treatment plan and document and bill OT services.
• Perform all other duties to effectively treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement practices are followed.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure and certification

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
