Job summary

Evolve Therapy Services in Rochester, MN is hiring an Occupational Therapist for an in-house Skilled Nursing Facility. This role offers Full Time, Part Time, or PRN positions; you will plan and direct OT services to ensure high-quality resident care in a compliant setting.

You will screen and evaluate residents, develop and monitor therapy plans, communicate findings to families and the care team, and supervise COTA staff while ensuring accurate documentation and billing.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents and develop treatment plans with goals and methods to achieve them.
• Communicate results and plans to residents, families, and interdisciplinary team.
• Establish resident therapy schedules with team members and ensure proper documentation and billing.
• Oversee COTA and ensure documentation and treatments meet standards and reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
