Job summary

Evolve Therapy Services is seeking a passionate Occupational Therapist to join an in-house SNF in Suring, WI. You will plan, direct, and supervise OT services to residents, ensuring high-quality care and compliance with standards.

The role supports screening, evaluation, treatment planning, scheduling, documentation, billing, and supervision of COTA, working with an interdisciplinary team. Full-time, part-time, or PRN positions available with competitive pay and benefits.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish goals and methods.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in coordination with team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA supervision and ensure documentation and treatments meet clinical excellence and reimbursement standards.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN positions available

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications
