Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role plans, organizes, and directs OT services to ensure high-quality resident care in a facility setting.

The position involves screening residents, evaluating and developing treatment plans, communicating with residents and families, scheduling therapy, documenting and billing, and supervising COTA as needed to ensure clinical excellence and proper

Qualifications

• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.
• Ability to collaborate with interdisciplinary team.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish resident therapy schedules with team members.
• Document and bill for occupational therapy services.
• Perform duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement compliance.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
