Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Memphis, TN. Full Time, Part time, or PRN with a $3,000 sign-on bonus. Plan and direct OT services to ensure high-quality resident care while complying with standards and regulations.

Responsibilities include screening, evaluating, planning, coordinating schedules, documenting and billing, and mentoring COTA. Active licensure and ongoing CEU participation required.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to identify best OT interventions.
• Evaluate residents and develop short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents and families.
• Establish individual therapy schedules with the care team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform duties to effectively treat patients and maintain quality care.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time ($3,000 sign on!), Part time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
