Job summary

Evolve Therapy Services in Jellico, TN is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility. This PRN role focuses on delivering high-quality OT services in partnership with our facility team.

You will screen residents, evaluate and plan treatments, coordinate schedules, document care, and supervise COTA as needed to maintain clinical excellence and compliant reimbursement.

Qualifications

• Requires degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:
