Halcyon Home LLC is seeking a PRN Home Health Occupational Therapist to support geriatric patients in San Marcos, TX, and surrounding areas. You will assess, plan, and implement OT services, collaborating with caregivers to optimize daily functioning.

The role requires Texas OT licensure, CPR certification, and a minimum of one year in home health is preferred. Travel locally with flexible scheduling and ongoing professional development are expected.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• Active state OT license or equivalent per 42 CFR §484.115(f).
• Minimum one year home health experience preferred.
• CPR certification and strong interpersonal skills; knowledge of DME.

Responsabilités

• Perform ongoing clinical assessments to determine patient needs.
• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
• Assess and facilitate interventions for ROM, strength, coordination, and adaptive equipment.
• Develop and evaluate patient care plans with patients, caregivers, and representatives.
• Document clinical notes, phone orders, and other records promptly.
• Provide education and counseling to patients and caregivers.
• Supervise OT assistants and home health aides as needed.
• Apply infection control and standard precautions during care.
• Communicate effectively with physicians and healthcare practitioners.
• Meet continuing education requirements and demonstrate professional growth.
• Promote the Agency’s policies to ensure high-quality care.

• Perform ongoing clinical assessments to determine patient needs.
• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.

• Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
• Document clinical notes, phone orders, and other records promptly and accurately.
• Provide education and counseling to patients and caregivers.
• Supervise and guide occupational therapy assistants and home health aides as necessary.
• Apply infection control and standard precautions during patient care activities.
• Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
• Meet continuing education requirements and demonstrate professional growth and competency.
• Promote the Agency’s philosophy and policies to ensure high-quality care.

Environmental and Working Conditions:

• Work in patients’ homes under various conditions, with possible exposure to blood, bodily fluids, and infectious diseases.
• Ability to work a flexible schedule and travel locally, with occasional exposure to unpleasant weather.

Salary Information

$110 - $110Hourly Wage

Requirements

• Education: Bachelor of Science in Occupational Therapy.
• Licensure: Active state license as an occupational therapist or meets the requirements listed in 42 CFR §484.115(f). Military spouses with an equivalent out-of-state license are welcome.
• Experience: A minimum of one year of home health experience preferred.
• Skills: Proficient therapy skills according to accepted standards of practice, good interpersonal skills, CPR certification, knowledge of durable medical equipment.
• Software Knowledge: Familiarity with HomeCare HomeBase software is a plus.
• Transportation: Reliable transportation, valid auto liability insurance, and current driver’s license.
