Job summary

Interim HealthCare is seeking a Home Health Occupational Therapist in Topeka/Lawrence, KS to help patients regain independence in their homes. You will work within a home health team to implement individualized therapy plans aimed at improving daily living activities and safety at home.

As a Home Health OT, you will conduct assessments, set goals, and educate patients and families while providing compassionate, patient-centered care.

Qualifications

• Graduate of an accredited Occupational Therapy Program and active OT license in Kansas.
• CPR certification required.
• Knowledge of state and federal home health regulations preferred.
• Strong clinical judgment and interpersonal skills; compassionate approach to patient care.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Collaborate with home health team (RN, LPN, CNA, HHA, PT, SLP) on patient goals.
• Assess patients, establish therapy goals, and document progress.
• Assist patients with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment to improve safety and independence at home.
• Educate patients and families on plan of care and self-care techniques.

As a Home Health Occupational Therapist, here's a big-picture view of what you'll do:

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

A few must-haves for Home Health Occupational Therapists:

• Graduate of an accredited Occupational Therapy Program and active OT license in Kansas
• CPR certification
