Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver high-quality home health services for adults in Modesto and surrounding areas. The role is an Independent Contractor position with the autonomy to set a schedule and manage patient care in homes.

You will assess functional status, create personalized treatment plans, and work with clients and families to improve daily living and safety. Bilingual candidates are a plus and state OT licensure is required.

Qualifications

• Ability and enthusiasm in working with patients of all ages.
• OT license and state registration.
• Completion of an accredited OT program.

Responsibilities

• Assess patients to determine functional status, ADLs, and home needs.
• Develop and implement OT treatment plans to improve wellbeing and safety.
• Evaluate patient progress and adjust plans as needed.
• Collaborate with families, caregivers, and care team to support therapy.

Job description

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Modesto and the surrounding areas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
