Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care in the Lancaster/Palmdale area. The OT assesses daily living activities and home functionality, develops a tailored treatment plan, and conducts interventions to enhance the patient’s wellbeing.

You will lead therapy in collaboration with caregivers, observe progress, and adjust plans based on state guidelines and patient response. This role offers flexibility and independent practice.

Qualifications

• Ability and enthusiasm in working with all ages.
• Licensure and state registration as Occupational Therapist.
• Completion of an accredited Occupational Therapist program.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient care needs related to functionality, ADLs, and home safety.
• Develop and implement a treatment plan; perform interventions to improve wellbeing.
• Monitor progress and adjust care as needed; document outcomes.
• Instruct patients, families, and care teams on therapy activities.

This position is for an Independent Contractor to serve in Lancaster/Palmdalearea specifically.

FeldCare Connectsis currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
