FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care to adults of all ages in the Palmdale area. This independent contractor role focuses on assessing daily functioning, planning individualized therapy, and supporting patients in their home environments.

You will lead therapeutic activities, document progress, and collaborate with a care team to ensure high-quality outcomes. Flexibility and independence are emphasized to empower clinicians.

Qualifications

• Ability and enthusiasm in working with all ages.
• OT license and state registration required.
• Completion of an accredited OT program.
• Bilingual ability is a plus.

Responsibilities

• Assess and evaluate patient care needs related to functionality, ADLs, fine motor, home assessments and adaptive equipment.
• Develop treatment plans and implement interventions to improve wellbeing.
• Evaluate patient progress and adjust care as needed.

This position is for an Independent Contractor to serve in Palmdalearea.

FeldCare Connectsis currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual a plus!
