Job summary

Trilogy Health Services, LLC is seeking an Occupational Therapist for our Warsaw, IN location. The OT will assess, plan, and implement therapeutic activities in an interdisciplinary setting, while educating patients and families and documenting progress to adjust treatment plans as needed.

Responsibilities include collaborating with a care team, ensuring quality patient care, and performing other duties as assigned. Travel is minimal and standard licensure is required to practice in Indiana.

Qualifications

• Master's degree in Occupational Therapy required.
• Active OT license in Indiana required.
• CPR certification preferred.
• 0–1 year of experience in OT is noted.

POSITION OVERVIEW

The Occupational Therapist (OT) is responsible for providing a full range of occupational therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Implements therapeutic interventions to improve patients' functional abilities.
• Educates patients and their families on techniques and strategies for daily living activities.
• Documents patient progress and adapt treatment plans as necessary.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree
Experience: 0-1 years

Licenses and Certifications:

• Must have and maintain a current, valid state OT licensure or registration
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
