Job summary

Saber Health Group in Minnesota seeks a licensed Occupational Therapist to join our skilled nursing facility therapy team. You will evaluate residents, develop treatment plans, and guide patients to regain independence.

You’ll educate patients and families, coordinate with physicians and nursing staff, and help residents return to optimal function through therapeutic activities and an individualized exercise program. This is a full-time, on-site role with competitive pay and CEU access.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy from an accredited college or university.
• Must possess a current and valid license granted from state of employment.
• Must possess basic computer skills and the ability to navigate through various software programs, including service delivery software and the electronic health record software.

Responsibilities

• Return patient to optimal physical function and facilitate achievement of therapy goals within a SNF setting.
• Instruct the patient in maintaining achieved level of function after discharge through education and independent exercise program.
• Coordinate with physicians and therapy team to implement individualized treatment plans.

Position Summary:
