Alternate Solutions Health Network is seeking a licensed Occupational Therapist to provide home-based therapy under physician orders in the Dearborn area. You’ll collaborate with an interdisciplinary team to deliver compassionate, quality care and help patients regain function.

You’ll educate patients and families, complete accurate documentation, and ensure safety while adhering to Medicare guidelines. A valid state license and driving is required; one year of acute care OT experience

• Licensed Occupational Therapist in the relevant state.
• Minimum one-year OT experience in an acute care setting preferred.
• Home health experience is a plus.

Responsabilidades

• Plan of Care: provide skilled OT per physician orders.
• Evaluate patient functioning and progress.
• Improve range of motion, strength, and coordination via PT/OT activities.
• Educate patients and families on disease, treatment, and home exercise.
• Supervise COTA and document care per regulations.
• Communicate findings with the care team and complete required documentation.

You will educate patients and their family members and ensures the safety of the patient.You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.Develop an individualized OT plan of care and make adjustments as needs change.Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.Supervision: Oversee and supervise total care of patient provided by COTA.Communication/Collaboration: Communicate with the COTA on supervisory visit results.Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:Compassionate communicator with a positive attitudePatience is a virtue when working with patients, families, physicians, and coworkersAttention to detail is critical, as is being observant and following directionsREQUIREMENTS:Occupational Therapist with a current license in the state of employment.Minimum one-year experience as a OT in an acute care setting.
