Schedule: 3 days – Mon, Tues, Wed, 8am–5pm. Agency: Beaumont Home Health.

You'll provide home-based therapy care as directed by the attending physician and interact with patients' families, experiencing the rewarding privilege of being part of every step of their recovery journey.

HOW YOU'LL WORK

You will provide comprehensive skilled services as ordered by the attending physician, educate patients and their family members, ensure patient safety, complete accurate documentation, and remain compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY
  • Plan of Care – Provide skilled occupational therapy as ordered by the attending physician; observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff; develop an individualized OT plan of care and adjust as needs change.
  • Evaluation – Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes – Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Minimize Patient Risk – Teach the patient preventative measures and decision‑making to decrease re‑hospitalizations.
  • Family Educator/Advocate – Counsel patient and family on disease/injury and how to manage through all phases of occupational therapy; instruct on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervision – Oversee and supervise total care of patient provided by a Certified Occupational Therapy Assistant (COTA).
  • Communication/Collaboration – Communicate with the COTA on supervisory visit results.
  • Policies – Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations – Understand and follow agency policies, procedures, rules, and regulations; communicate changes in schedule/availability to schedulers or supervisors.
  • Operations – Attend in‑service training and mandatory agency meetings.

REQUIREMENTS
  • Occupational Therapist with a current license in the state of employment.
  • Minimum one‑year experience as an OT in an acute care setting; home care experience preferred.
  • Valid driver’s license and auto insurance in the applicant’s name as a driver.
  • Capable of all physical demands required by the position.
