Job summary

EASTERN HEALTH GROUP in Roanoke, Virginia, is seeking an Occupational Therapist (OT) to join our rehabilitation team. You will assess referred patients, provide direct care, and help design goal-directed treatment plans that fit each patient’s capacity and tolerance.

This role emphasizes collaboration across disciplines, documentation, patient and family education, and adherence to HIPAA and Residents’ Rights, with a focus on resident-centered care in long-term care and geriatric settings.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and potential for rehabilitation.
• Establish and administer a treatment program with goals determined by patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families in an appropriate therapy method.
• Perform evaluations, weekly progress notes, re-certifications and discharge documentation.
• Collaborate with all disciplines to provide patient-centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPPA and Residents Rights compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition

• Educate patients and families in an appropriate therapy method.
