Job summary

Eastern Health Group in Roanoke, VA seeks an Occupational Therapist to evaluate and treat residents in skilled nursing and rehabilitation centers. You will conduct assessments, create individualized treatment plans, and collaborate with the care team to promote best patient-centered outcomes.

Requirements include graduation from an accredited OT program, valid licensure, and preference for experience in long-term or geriatric care.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Licensed and registered to practice OT in the state of practice.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals determined by patient capacity under physician direction.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families about therapy methods.
• Complete evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Perform duties assigned by the Director of Rehabilitation.
• Ensure confidentiality, HIPAA compliance, and patient rights.

Duties And Responsibilities

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
