Job summary

Eastern Health Group is seeking an Occupational Therapist (OT) to join our family of skilled nursing facilities and rehabilitation centers. The OT will assess patients, develop and administer rehabilitation plans, and collaborate with a multi-disciplinary team to achieve patient-centered outcomes.

Candidates must have an accredited OT degree and a valid license/registration for the state of practice. Experience with geriatric or long-term care is preferred.

Qualifications

• Requires graduated Occupational Therapist from an accredited program.
• Requires valid professional license/registration for practice state.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate medical condition and functional capacity.
• Establish and manage a treatment plan with physician-guided goals.
• Document patient status and progress.
• Educate patients and families on therapy methods.
• Complete evaluations, progress notes, re-certifications and discharge docs.
• Collaborate with interdisciplinary team for patient-centered care.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
