Job summary

Aventura Health Group is seeking an Occupational Therapist (OT) to evaluate residents, develop treatment plans, and help restore independence in ADLs within our skilled nursing and rehabilitation facilities.

You will collaborate with an interdisciplinary team, educate families, document assessments and progress, and ensure compliance with HIPAA and residents' rights. A valid OT license is required; geriatrics experience is preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Valid OT license and registration for the state of practice.
• Experience in long-term care or geriatrics is preferred but not required.
• Strong communication and organizational skills.
• Compassionate, team-oriented, and committed to resident-centered care.

Responsibilities

• Evaluate referred residents to determine functional level and rehabilitation potential.
• Develop individualized treatment plans to help residents regain independence in ADLs.
• Establish treatment goals based on abilities, limitations and physician recommendations.
• Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills.
• Educate residents and families on home programs and adaptive techniques.
• Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Collaborate with the interdisciplinary team to ensure a resident-centered approach to care.
• Adhere to applicable federal and state regulations, including HIPAA and Residents' Rights.
• Perform additional duties as assigned by the Director of Rehabilitation.
