Job Summary:

We are currently seeking a passionate and dedicated PRN Occupational Therapist (OT) to join our rehabilitation team. The ideal candidate will deliver exceptional therapy services to residents in accordance with individual care plans and facility standards.

Responsibilities:

• Evaluate residents’ functional status and establish individualized therapy plans.
• Deliver occupational therapy treatments to improve residents’ abilities in daily living and functional skills.
• Collaborate with nursing staff, physicians, and family members to ensure comprehensive care.
• Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes.
• Participate in interdisciplinary meetings and contribute to discharge planning when appropriate.
  
  
  

Qualifications:

• Current and valid Occupational Therapist license in the state of PA
• Graduate of an accredited Occupational Therapy program.
• Prior experience in a skilled nursing or long-term care setting preferred.
• Strong communication and interpersonal skills.
